(３)Documents confirming the identity of the representative, if the application is made by a representative.
If the client’s representative makes the disclosure request, please include a document confirming the representative’s identity (a copy of one of the previous items) and a letter of authority (with the person’s seal).

Responses to your request for information will be sent by registered mail to the address of the data subject. In the case of a request from a representative, it will also be sent to the address of the person to whom the personal data relates.
